Request An Appointment

Thank you for your interest in Community Dental. To better direct your request, please answer the following:

    Are you experiencing pain, swelling or infection?

    We want to help get you out of pain as soon as possible. Whether you are a new or existing patient, please call or visit the Community Dental center nearest you and speak with our staff directly.











    New and existing patients can request an appointment online!

    Select a Community Dental location from the list to be taken to that center's appointment request page.

    New patients: Be sure to bring your completed patient information packet to your appointment. You also have the option of mailing it to the dental center prior to your appointment.

    Existing patients: You will need to complete the patient information packet if:

    • You have had any changes to your health, such as surgeries, hospitalizations, new medications, etc.
    • You have moved or have a new mailing address and/or phone number
    • Your insurance has changed or is no longer active (if your insurance has changed, please provide a copy of your insurance card as well).
    • You would like to apply for our income-based sliding fee discount. You will need to provide income information.

    You have the option of mailing your updated information to the dental center prior to your appointment.
