of the Website. By using the Website, you signify your assent to these Terms. If
you do not agree to all these Terms, do not use this Website.

Our server does not recognize visitors' e-mail addresses and cannot personally identify
anyone unless personal information is given. We do set cookies. Cookies refer to

with this entity, as well as retained by Community Dental. Other than acquisition
or sale, Community Dental does not sell or rent your email address or any other
information to unrelated third parties. The information we collect is used to improve
the content of our Website. It is only shared with Community Dental and its affiliates.
THIRD PARTY COOKIES
Be aware that Community Dental may use third parties to assist us with this Website.
Some examples of the assistance that may be provided by third party service providers
are: fulfilling orders, processing payments, monitoring site activity, providing
fraud prevention services, conducting surveys, administering and sending mobile
messages, and administering emails.
This Web site uses Google Analytics (GA). GA is a free service offered by Google
that generates detailed statistics about the visitors to a website. GA can track
visitors from all referrers, including search engines, display advertising, pay-per-click
networks, e-mail marketing and digital collateral such as links within PDF documents.
Many ad filtering programs and extensions (such as Firefox's Adblock and NoScript)
can block the GA tracking code.
FACEBOOK AND GOOGLE+
When you click the Facebook "Like" button on a Community Dental Webpage [this requires
you to be logged-in to Facebook or will prompt you to log-in], you are making a
connection to Facebook. This connection creates a Cookie with a unique user ID that
allows Facebook to track your movements on the Internet. The Facebook cookie is
not only sent when a member wants to log on to Facebook, it is also sent every single
time a Web site which includes the 'Like' button is visited. Data about the user
is sent to Facebook regardless of whether the Like button is actually activated.
Information in your Facebook profile may be accessed and collected by Community
Dental if you chose to allow us to access and collect information from your Facebook
account through our Website. “Liking” Community Dental and logging-in to Facebook
triggers access. While Facebook allows access to more categories of information,
Community Dental may only collect the following data: your name, profile picture,
gender, friends, likes, education history, work history, current city, hometown,
interests, relationship status, and any other information you have shared with everyone.
It is possible that Community Dental will be able to continue to collect this information
from your Facebook account should the information be revised by you later.
When you click the Google "+1" button on Community Dental Website, you are making
a connection to Google. This connection creates a Cookie with a unique user ID that
allows Google to track your movements on the Internet. When you +1 your identity
is known if you log-in to the Google+ Project [Google’s Social Network]. The fact
that you +1’d something will be recorded by Google, along with information about
the page you were viewing when you clicked on the +1 button. Your +1’s may appear
to others as an annotation with your profile name and photo in Google services (such
as in search results or on your Google Profile) or elsewhere on Websites and ads
on the Internet. +1’d pages do not appear only on the +1’d Tab of your Google Profile
but also they are visible when one of your friends is searching for a relevant topic
on Google. You may opt out of seeing +1 recommendations on third-party Websites
(including on ads on third-party sites) from people you know.
This Privacy Policy does not cover the privacy and security practices of Facebook
or Google+. Please review the Facebook and Google+ privacy policy and terms if you

PAGES THAT COMMUNITY DENTAL HAS ON SOCIAL MEDIA SITES
Personal information may be collected directly by Community Dental through Social
Media Pages and may also be collected by the social media site hosting the Community
Dental Social Media Page. The collection of personal information directly by Community
Dental on its Social Media Pages will include the type of information addressed
under “Information Collected” above.
Examples of how personal information may be directly collected on Community Dental’s
Social Media Pages include providing information through an application or form
which has a link to this Privacy Policy. We will use and share the information we
collect on our Social Media Pages in the manner specified in this privacy policy.
This Privacy Policy does not cover the privacy and security practices of the Social
Media Sites on which Community Dental has Social Media Pages. Please review privacy

The social media sites where Community Dental has its Social Media Pages may provide
aggregate information and analysis to Community Dental about visitors’ use of our
Social Media Pages. This allows us to better understand and analyze our user growth,
general demographic information about the users of our Social Media Pages, and interaction
with content on our Social Media Pages. Overall, this information may be used to
help us understand the types of visitors and users of our Social Media Pages and
use of the content.

Communitydentalme.org does provide links to other sites. Other Internet sites and
services have separate privacy and data collection practices. Once you leave Communitydentalme.org,
Community Dental has no control or responsibility over the privacy policies or data
collection activities at another site. Further, Community Dental does not endorse
or sponsor the sites to which it has established links.
SHARING OF PERSONAL INFORMATION
We do not partner with or have special relationships with any ad server companies.
From time to time, we may use this volunteered information for new, unanticipated
uses not currently disclosed in this privacy policy. If our information practices
change at some time in the future, we will contact each individual before we use
his or her data for these new purposes and allow each person to opt out of these
new uses. We will also post the policy changes to our Website to notify visitors
of these changes and provide visitors with the ability to opt out of these new uses.
If you are concerned about how your information is used, you should check back at
our Web site periodically.
Companies may be engaged by Community Dental to perform a variety of functions,
such as, assisting with donation collection, providing technical services for our
web sites, etc. These companies may have access to personal information if needed
to perform such functions. However, these companies may only use such personal information

Other than as provided in this privacy policy, Community Dental does not sell, transfer

site, to fulfill your requests, or to cooperate in any law enforcement investigation

will be listed as a donor in our annual report unless you request to be removed
from this listing.

at the above address. These requests will be processed within two weeks of receipt.

When we transfer and receive certain types of sensitive information, such as financial
